A Note on Changes to the Patient Portal:
On September 30, the Aprima portal will no longer be available for log-in or use.
The Aprima Patient Portal contained information about provider visits that happened on or before January 20, 2018. Your health information for provider visits prior to January 20, 2018 (including a list of your appointments, medications, lab results, and visit summaries), has been transferred from the Aprima portal to your eCW portal account. Any information not transferred is available in the CrescentCare medical record.
If you saw your provider on or after January 22, 2018, visit information will continue to be found in the eCW patient portal.
If you cannot locate what you need in the eCW portal, you may request a copy of your medical record by filling out the Patient Authorization to Release form and sending it to CrescentCare. You can also request your record in person by visiting any CrescentCare location.
